Upload Your Certificate of Completion and Letter of Completion from the 501(c)(3) Organization

Please Read This Carefully - Community Service Option

This form allows the violator who has qualified for the community service option to upload their certificate of completion and their letter of completion from the 501(c)(3) with which community service was performed.

  • Once you have completed your Defensive Driving Course, you must submit your certificate of completion to Diversion Management to fulfill your obligation to the Traffic Diversion program.

    Please note, you can not use a certificate with a completion date prior to the date your violation occurred.

    When you registered for the Diversion Program, you received an email acknowledging your registration. That email included an Application Identification Number between two (2) and eight (8) digits.

    Please carefully enter the Application Identification Number below.
  • THIS IS NOT YOUR LICENSE ID OR CITATION NUMBER. IT CANNOT BE FOUND ON YOUR CERTIFICATE.

    You can find the Application Identification Number in the email that was sent to you when you were officially accepted into the Traffic Diversion program.

    The subject line of the email reads: "IMPORTANT -- DO NOT DELETE: Madison County Traffic Diversion Program Follow-up"
  • DO NOT SEND A SCREENSHOT OF THE COURSE WEBSITE AS PROOF OF COMPLETION. YOU MUST SEND AN ELECTRONIC VERSION OF THE ACTUAL COMPLETION CERTIFICATE. A PDF VERSION OR A PHOTO OF THE ACTUAL CERTIFICATE IS ACCEPTABLE.
